    Summary: Mrs X complains about the Council’s handling of a complaint she raised on behalf of her two children, Y and Z, investigated under the children’s statutory complaints procedure. The Council is at fault. It has agreed to re-take the best interest decision related to Z, pay Mrs X £600 in recognition of the distress caused to her and Y by lost unsupervised family time and improve its services.

    Summary: Ms X complained the Council failed to provide suitable education to her child, Y, while she was absent from school. On the evidence seen, we found fault with the Council for failing to provide suitable Alternative Provision of education for Y. The Council agreed to apologise to Ms X and Y and pay them £1,400 for the missed education. The Council also agreed to review its policy/mechanism for how schools inform it of the details of pupils who fail to attend school regularly.

    Summary: Ms X complained the Council took too long to complete the annual review of her son’s Education, Health and Care Plan. We found fault by the Council. The Council agreed to provide Ms X with a written apology and a payment for the distress caused to her and to improve its service.

    Summary: Mrs X complained about the quality of home care provided to her by the Council’s commissioned care provider, Choices Homecare. She also said the care provider failed to properly address her complaint. We find the Council was fault. This caused distress to Mrs X. The Council has agreed to several recommendations to address the injustice caused by fault.

    Summary: Ms Y complained the Council failed to provide all the special educational needs support set out in her child’s Education Health and Care Plan and delayed completing the annual review of the plan. We have found fault by the Council causing injustice. The Council has agreed to remedy this by apologising, reporting to us on progress, making payments and service improvements.

    Summary: Miss Y complained about the way the Council dealt with her homelessness application and repair issues at her accommodation. We have not found fault with the timescale in which the Council accepted the relief and main housing duties, or with its response to the repair issues. We have found fault by the Council in failing to consider the location of Miss Y’s daughter’s school and record its assessment of the suitability of the interim accommodation it offered Miss Y, causing injustice. The Council has agreed to remedy this by apologising to Miss Y, making a payment to recognise the distress and worry caused, and service improvements.

    Summary: Mrs X complained about the Council’s failure to share a stage two report with her following a children’s statutory complaints procedure investigation and for the failure to carry out the agreed actions from a previous investigation by us. The Council was at fault for the failure to properly consider what aspects of the report could be shared with Mrs X and for the delay in carrying out the recommendations from our previous investigation. It has agreed to apologise and pay Mrs X £500 to acknowledge the distress and frustration caused and to retake the decision on what aspects of the report it can share, considering all relevant factors.
