    Summary: Mr Y was a vulnerable man who died during a hospital admission. We found fault on the part of two NHS Trusts concerning their failure to work together to identify Mr Y’s family. This meant Mr Y’s funeral proceeded without their knowledge. We also found fault by one Trust regarding the handling of a safeguarding referral for Mr Y. This fault caused Mr Y’s brother, Mr X, significant distress. The NHS Trusts will apologise to Mr X and pay a financial remedy in recognition of the impact this fault had on him. These organisations, along with the Council, will review their policies and procedures to prevent similar problems occurring in future.

    Summary: Mr D complains about the way his mother, Mrs E, was discharged from hospital, and that communication with him was poor. The Trust accepted its communication with Mr D about Mrs E’s condition while she was in hospital was lacking, meaning he did not have all the relevant information. The Trust has apologised to Mr D and taken steps to improve communication. We did not find fault by the Trust or Council with regard to the other matters we investigated.

    Summary: The Ombudsmen will not investigate a complaint about the way a care home requested a repeat prescription or prevented access to the home for a planned advocacy visit. This is because there is no evidence of significant injustice to the resident, and the injustice to their family has already been remedied.

    Summary: Mrs A complains about a council and care home relating to the care and treatment provided to her mother in 2020. We found fault in fluid management recording and in staff not seeking emergency treatment earlier. The Home has taken appropriate action to address these issues in its care of other residents, but we have recommended it apologises for the uncertainty caused to Mrs B for the delay in seeking emergency treatment.

    Summary: The Ombudsmen have decided not to investigate Mr X’s complaint about a decision to discharge his mother from hospital into a care home. This is because there are no indications of fault by the Council or the NHS Trust. At the time, measures were in place due to the COVID-19 pandemic and the decisions appear to have been in line with government guidance.

Also, Mr X has already used his appeal rights to challenge Westminster City Council’s decision that he was intentionally homeless.

    Summary: Miss B and Mrs C complain about the care their late grandmother Mrs P received in a council-funded care home and from district nurses. Failings in Mrs P’s care by the Home and district nurses led to her developing severe pressures ulcers in the last weeks of her life. The Council did not investigate key issues in the complaint. The organisations should apologise and make a payment to Miss B and Mrs C to acknowledge the distress they suffered. The Council should also ensure the Home makes service improvements.
